Early in the US space program, many of the astronauts complained of their urine leaking out into their space suits. The system was pretty simple; they wore a condom catheter on their dicks which drained into a pee bag, but the engineers got to work trying to figure out what the problem was.
Turns out, they had just been asking what size catheter condom the astronauts needed, and every one of them had asked for the largest size. After they started measuring the dudes' dicks before sending them up, they stopped having problems with leakage.
