Haven't been posting much partly cuz I'm trying to not be distracted while I'm finishing the game but also because I've been coping with GERD-like symptoms since roughly October of last year and it's been pretty distressing! I haven't been in AGONY since my initial flare-ups but there's something degrading to one's mood & sense of optimism about having spans of days where your periods of actual full-body comfort can be measured in minutes. Over-the-counter medication has been helping but not to such a degree that I'm confident it'll be a sustainable solution long-term, so I'm going to talk to my MD this week about getting some tests done and possibly even getting a referral to a gastroenterologist. I'm in good health otherwise and am doing my best not to catastrophize, but it just Sucks to be staring down the barrel of a potentially-life-altering chronic condition at age 34. Oh man, that sucks to hear, bud. :( My wife has been on the Low FODMAP diet for a long time for her own gastrointestinal issues. And yes, life after 30 is often like an RPG where you just randomly pick up a chronic condition which really sucks!!

Yeah I'm 6/8 weeks through a self-prescribed course of esomeprazole (20mg/day), supplemented occasionally by famotidine and/or Diavol. It's keeping me going but symptom reduction so far hasn't been so complete that I'm confident things won't worsen quickly once I'm through the course

Yeah I'm not necessarily opposed to needing meds (I'm open to upping my dosage if the MD recommends), but I wanna run some tests just to rule out potential underlying conditions like h. pylori infection or hypochlorhydria. I'm also very open to surgical interventions if that's on the table at all (the LINX procedure just SOUNDS cool in addition to apparently giving good results)

Yeah definitely pursue all avenues, not trying to be discouraging or anything, I was disappointed when my own gerd diagnosis came up negative. There's just something wrong with me that runs in the family, but if you can get your stuff fixed my understanding is they've made some hella advances and I hope that's how it works out!

Well my mom also has reflux issues (which she mainly controls with Diavol & diet) so it's not the MOST surprising thing. I'm guessing you & your dad haven't really had any trouble with side effects from the long-term PPI use?

Yeah to the best of my knowledge my dad hasn't had any problems, I've discussed this with my mom and she never mentioned him having any side effects, and that's the sort of thing she would bring up I think. My doctor at one point warned me that there can be long-term issues, and that worried me for a while, but I asked her about it again eventually and she said that all she'd meant was that under some circumstances, the decrease in stomach acid can cause partial digestion and resulting malnutrition, but that she didn't think that was likely. So as far as I know you can just take this stuff forever.

I was just reading this large-scale meta-analysis of many meta-analyses focused on assessing the risk factors associated with long-term PPI use: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6463334/ The meat-and-potatoes is that there's a decent link between longterm PPI use and somewhat-lowered magnesium levels, as well as a decent amount of evidence showing increased risk of chronic kidney disease, but the evidence of most of the other commonly-cited adverse side effects appears to be "low to very low", and the authors conclude that the potential risks pale in comparison to the overall societal benefit of the drugs. Comforting stuff!
