Cursor keys:

Precise

Very slow

Page up/Page down keys:

Very fast

Imprecise

Scrollwheel:

Very fast
Very precise

Have to move hand to the mouse

Scrollwheel 1cm left of Tab key:

Very fast
Very precise
It's right* there

IT JUST MAKES SENSE

any good keyboard is also large. why not also have one for scrolling horizontally? you could even make that into a scroll-ball.

i would love an addition to the left of the keyboard for that. would be very convenient

It's not quite the same, but it's not uncommon for hand-built mechanical keyboards to have rotary encoders that act as scroll wheels (or volume controls, etc)! Seconding the rotary encoder! I think there are kits that let you place it near the escape button, which might be nicer for middle/index finger usage.
