I like how super-power inducing accidents are so common in super hero comics, and yet no one seems to have more than one.

We can call her Accident-Prone Woman.

"Oh darn, I fell in a vat of toxic waste and got barf powers"
"Ah heck, I got stung by a radioactive bullet shrimp and now I have all the powers of a bullet shrimp"
"Oh gosh, I was chosen by the God of Boring Television and now I can put people to sleep"

Are superpower-inducing incidents randomly distributed? Or, are some environments or lifestyles more likely to put a person in the path of an SPII? Are the changes that come with your new powers protective against further SPIIs (e.g. spider sense that helps you avoid future spider bites)?

This kind of thing sorta does happen in comics - generally due to a character just being around for a real long time and various creators trying out new stuff with 'em. Carol Danvers would be the best example, cycling into and out of various mishap- and or experimentation-induced powers over her long publishing history (by my count at least three times by the mid-80s, and god knows how many more since).
