A trucking company involved in a fatal collision in Ohio had previously been cited for issues like defective brakes, according to documents obtained by ABC News. The crash occurred when a Mid State Systems truck rear-ended a car, leading to a chain-reaction collision involving a coach bus and another truck. The bus was carrying high school students, three of whom died. The documents show that the trucking company and the other truck involved in the crash had multiple violations related to brake defects and other issues. The National Transportation Safety Board is currently investigating the incident. No charges have been filed yet.
