Unsure if the credit information on this is correct; it was embedded in the metadata of this WAV file. Wait, WAV files can have metadata? In 1993? Hmm...

File also contains a Comment field reading "Tutorial file: "Ouch, that hurts" for Sound Forge 2.0." and a copyright field reading "Copyright Cambium Development, Inc. 1993. All rights reserved."

Warrants further investigation at some point, I suppose - I found it in a Microforum software catalog, on Game Empire Vol.2 CD-ROM.

WAV files not only can have metadata, said metadata is how the sound samples in Quake know where to loop from instead of always looping from the beginning. (Somewhat frustratingly, Audacity doesn't expose this metadata... but I believe foobar2000 does.)

Not only can WAV files have metadata, Quake actually uses metadata in its WAV files to determine where to loop samples that are capable of such. Annoyingly, Audacity does not expose this metadata for editing.
