Back in the day when I ran The Furry Survey, @indilatrani made something similar! An interesting thing about the UCSF version is that I bet the smaller squares in a grid lead to a lot less priming along the axes versus this free-form version — and, as Indi pointed out, it's likely also more accessible for that.

My best guess is that someone consulted with their one (1) queer friend about the best way to handle gender, and that friend decided to do The Funniest Possible Thing.
Either that or the program is legally required to ask the question and is committing malicious compliance.
