Usual thing to ask is 'are you ok'. Implies a binary yes no response, and some may feel a social pressure to reply 'I'm fine'.
'What is your relative level of ok?' - not binary, invites the respondent to supply more complex information about their state, hopefully with the understanding the questioner sincerely wishes to know and understand?
