personally I would consider everything described above as “outside,” based on the notion that a pigeon or a coyote could just waltz into any of these places.
I don't know if that's necessarily a fair metric to use, given that both those animals are notorious for getting places they shouldn't be
underground metro stations are outside because you can vape in them without bothering people
outside -> inside: when your elevator passed below street level. inside -> outside: when your train exited the tunnel mouth on its way to the aboveground stop. the interior of the train does not count in terms of locating you as either "inside" or "outside" because, the train is an inherently mobile and impermanent means of conveyance between (relatively) permanently-located spaces; "inside" as in a vehicle is a different concept entirely than "inside" as in a subway tunnel.
