the austere/hedonist quirks don't really work for the good doctor, because his philosophy is fundamentally hedonistic, but his idea of pleasure is not food/drugs/sex/whatever. rather, the things he takes pleasure in fall more along the lines of "drink and attempt to metabolize a poison that would kill most people stone dead," or "write a theorem in eldritch math that might set your house on fire," or "turn some organs inside out, just because." maybe all three of these at the same time
