(fallen london stamps by the austere/hedonist quirks don't really work for the good doctor, because his philosophy is fundamentally hedonistic, but his idea of pleasure is not food/drugs/sex/whatever. rather, the things he takes pleasure in fall more along the lines of "drink and attempt to metabolize a poison that would kill most people stone dead," or "write a theorem in eldritch math that might set your house on fire," or "turn some organs inside out, just because." maybe all three of these at the same time
it really doesn't help that he's also a workaholic for Coping and Possibly Undiagnosed Bipolar reasons and his lines of work often involve these things
