it's working now and it was only one question which was "would you like to answer some questions about your exercise habits" and i said no and that was the entire survey
wow they did the exact same thing again for the allergist appointment that the regular doctor referred me to. i feel like someone high up in Kaiser must have been like "yes we need Metrics on Exercise, better have an alert come up before every single appointment to ask people if they want to give us free information on them"
