fun and exciting VS Code issue: if something detonates internally in an extension, that extension can be so dead it doesn't have commands in the command palette. this can be frustrating if the extension is a shitty LSP extension and you know that restarting the server fixes weird issues but the "restart the server" command doesn't exist because the extension died.
