A while back I noticed Final Burn Neo added support for the Sega C-2 aka Puyo Puyo 2 arcade:

seems there’s been work in progress the past few months in adding emulator support for sega c-2 arcade hardware in final burn neo. this means fightcade support for puyo puyo 2 arcade could become a reality in due time. also columns 2 I guess for all the columns fans out there (?)

I guess I didn't realize until recently that the Fightcade devs had no interest in updating the core FBNeo engine any time soon, so I took it upon myself to see how much work it would take to add the C2 driver myself, and lo and behold, it wasn't much! Submitted a pull request today; we'll see how long it takes to have it accepted and released.

Not too familiar with this… what are the major differences between this and the MD version on the multiplayer side (other than the obvious prestige of having the authentic "arcade experience")?

The full audio is definitely the big one, game flow is also improved since you can turn on Free Play so you don't have to menu through the game over screen every time match point finishes. I also feel better about making rom hacks for a "definitive" version of the game
