Feeling bad for Tara Strong and James Urbaniak who lend their VO for this already hacknyed setup, which precedes to be run into the ground and spat on. Not that the trolley problem itself was funny but rather the memes spawned out of it.
Some observations

none of the writing is very good

if the player chooses to do nothing, the remaining guy admits that he tied himself to the tracks for fun which means the entire thing is moot. One of the five run over by the trolley admits to doing genocides.

if the player flips the switch and kills one guy, the character who admits to genocides says he's going to do more.

the player character has a knife, the trolley moves incredibly slow, it would be trivial to remove everyone from the scenario before the trolley reaches either tracks.

this is to date the longest exposure I've had to High On Life and if it's any indication on the rest of the game, well, I guess we're in for more of This Shit Forever, since it's a top seller on Xbox Game Pass and Steam. Justin Roiland has ruined tv animation and is now set to ruin videogames.
