i'm also investigating an issue i saw last night where sometimes certain notifications wouldn't show at all (most noticeable with comments). i think it's just an issue with pagination that means they're stuck in a void between pages but we'll see! it wasn't that but i figured it out and it will be in the batch today.
best of luck to ya! also for the record i'm fairly certain it doesn't only happen between pages -- i've had a comment notif get eaten that would have been squarely in the middle of page 1
good to know! part of my difficulty debugging it is that i can't always tell if someone is actually mid-page, since we're hoisting up notifications from the bottom of the page to Wherever. but this is helpful!
