I think it's because Bluesky aims to be "federated" which is what Mastodon is. The long and short of it meaning it's more of a messaging protocol where, in theory, you could be bsky.app/profile/joewintergreen.games or whatever because there's a federated social media platform at .games somewhere.
There may be an easier way to set that up because I already see people on Bluesky doing stuff like that, like this guy somehow gets to be tiny.baby:

It's possible more custom URLs are only available for "high ranking" (verified?) users right now, idk. I don't use Bluesky enough to dig into whatever is going on here besides knowing it's something I cannot do
Literally anyone can pick a custom URL if they own it, as I understand it just works by hosting a particular file at the right place at that domain
