I'm All out of Health is an interesting take on the sickly female lead fantasy romance. There's a fair few of these, and typically they seem to either go for the like victorian style elegantly and beautifully and tragically fading away (see: Karina's Last Days/Last Days of the Terminally Ill Extra, where the protagonist is dying of being too good at art1) or else play it more for laughs (see: Happy Ending for the Time-Limited Villainess/The Villainess's Days Are Numbered, where the character has a health bar going down the whole time basically).

I'm All Out of Health seems to center more the profound inconvenience, indignity, and isolation of chronic illness. In the episode I'm on now, the protagonist has to decide whether to fly somewhere, knowing this is putting her life on the line, which is an all too real thing for lots of folks. (Although in this case it's flying using elemental magic lol.)

On the other hand, there's a lot of diet and exercise stuff, in lieu of actual medical care, which may just be because of the fantasy setting, but it's still a bit of a red flag.

The male lead is put in the role of reluctant caretaker, written as a traumatized germophobe with an aversion to human contact. This is a common trope in these romances generally (always overcome with the power of love/plot armor etc. etc.), but in this case it creates some interesting dynamics. I think there's also some problematic potential here, but again, interesting.
