Honestly might not be the worst idea. Back when mp3 players were real tiny there would be armband holders for them for when you were working out.
Maybe a collar-fit bluetooth DAC that outputs to some real short earbud cables could be functional. If you want better audio than full-wireless bluetooth buds but wanna minimize the amount of cables being run, you could probably trim back some earbud cables to not leave too much extra when going from your ears to the jack on your neck.

the Fiio BTR1K might be a good candidate actually. It's very compact for what it offers, and it comes with a removable clip. It's already designed to be usable clipped to the collar of your shirt and honestly that sounds more annoying than on an actual collar. Seems it'd bounce around a lot there if you were working out.
Could possibly just have a special pocket in the collar to slide the clip into nice and snug.
