The kanji transcription is from Uta-Net. The English lyrics are transcribed from the uncredited PGS subtitles on this encode, which I can only assume are from the US Blu-Ray. This is only the TV size portion of the song, but I might try translating the rest myself when I'm more confident in my abilities.
