I often think about how if you were truly sent to some fantasy world, what are the chances you get some virus in that world that your body is not adapted to so you just pass away from it. I mean in Nier humanity dies out because of the magic in the real world basically. Alternatively you yourself could be a bio weapon there like in Nier.
