Wrapped this up tonight and yep, it continues to be astoundingly high-quality. Never a better time to be a Quake fan. I might even have to go back and play the updated version of episode 1 too.
They've added a couple of new enemy/powerup/weapon types, and while I'm wary of Quake mods piling stuff onto the formula, their use here feels well-considered. Taking the Serious Sam kamikazes wholesale feels a little tonally weird, but they function quite well as gentler Spawns. The new 'rotary shotgun' serves an interesting role, with a damage output that's technically higher than the SSG but requires sustained tracking, and the 'crystal lance' serves as a railgun that can pass through enemies, rewarding smart positioning in hordes while also granting the player a long-range heavy-hitter for those big, cavernous, limit-removing maps.
I'm not sure all the maps are winners (I spent far too long wandering aimlessly around 'Blessed Dwellings in the Dark' looking for switches) but the first half is very strong, and the penultimate map is a ludicrous 700-monster saga with enough fresh ideas to fill an entire episode of its own. Even the final boss is pretty good—if you can excuse the fact that it asks you to shoot flying enemies while littering a load of hazards at ankle level.
