in the us they are typically depressingly dispatched from entirely separate private businesses. idk about anywhere else
here in Cambridge (UK) at least they're actually often just hovering around various spots when they're not actively in transit, distributed around town so they can get to various places quicker than if they were to park and wait at the hospital
