I've worked in a store where they had something setup like that. It wasn't using the phone's speaker, the handset cord was going to an amplifier.
What was horrible is that whenever the phone system rebooted, you had to go there with a broom and whack the receiver hook button until it registered as "off hook" in the PBX, then you could use paging again.
